Coverage ends

When your employment ends or when you are no longer in an eligible unit. The policy includes options to continue coverage on an individual basis when coverage as an active employee ends. See the certificate of coverage for details.

Guarantee issue During Initial Eligibility period
(first 60 days as newly hired employee or transfer into an eligible unit)

Employee may elect up to $250,000 employee coverage; up to $20,000 spouse/domestic partner coverage and $10,000 dependent child rider coverage without providing evidence of insurability (EOI)

Guarantee Issue during Annual Enrollment
|
Employees who are currently enrolled may elect to increase coverage by one increment ($10,000) if enrolled for less than $600,000 during annual enrollment without providing EOI. Employees who are not currently enrolled in Supplemental coverage must complete EOI for a new election.
If currently enrolled with spouse/domestic partner life coverage, employees may increase spouse/domestic partner's coverage by one $5,000 increment without EOI.
If not currently enrolled with spouse/domestic partner life coverage, you must complete an EOI for spouse/domestic partner's coverage.
Dependent child(ren) are covered by a rider insuring all eligible children. This coverage does not require EOI.
